Required Forms and Programs 
You may download any of the following Texas Real Estate Commission (TREC) real estate or home inspection forms by clicking the title of the desired form. Each of these forms requires Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, then simply download it here.
TREC encourages all licensees to file license applications electronically at the TREC website, otherwise a $20.00 processing fee will be applied to each license application submitted on paper.
Real Estate Forms
- SAE Cover Sheet – use when faxing or mailing SAE course completion certificates to TREC
- Application for Inactive Sales Agent License – TREC prefers that you fill out your application online. If that is not possible, then you may use this instead.
- Application for Broker License – TREC prefers that y9u fill out your application online. If you are unable to do so, use this.
- Moral Character Determination – If you have any criminal offenses, unpaid judgments, received discipline against a professional or occupational license, or have performed unlicensed activity, then you should considering requesting an MCD. The MCD process is significantly cheaper than paying for an application, examination, and the education necessary to receive a license.
The TREC requires that all education providers notify both current and future students of their right to request a criminal history evaluation.
*Before registering, please note that a criminal history may make you ineligible for a Texas sales agent license. You may request a criminal history evaluation prior to enrolling in a course or applying for a real estate license. You may refer to the Texas Real Estate Commission’s criminal history eligibility guidelines set out in the Texas Occupations Code 53.025 and TREC Rule 541.1. You may request a criminal history evaluation by submitting a completed Moral Character Determination Form to TREC prior to applying for a license.
